[Request] Slow cooker: Polish sausage, peppers and onions, potatoes, gravy

The cafeteria in my workplace whipped up an amazing lunch today: sliced/grilled Polish sausage with onions and green peppers, all over mashed potatoes and then topped with a light brown gravy.

What I'm looking for is a slow cooker recipe that I can make at home which will roughly approximate this meal. I'm looking to feed 3 to 4 people. I have a moderate comfort level in the kitchen but not a lot of experience and fairly limited funds at the moment. My hope is to be able to make this for my mom's birthday later this week, since it's the kind of meal she'd love.

The potatoes don't have to be mashed; cut up/cubed is fine, I mostly just want the core ingredients, especially the gravy. I did some googling and found a few recipes that sort of fit what I'm looking for, but they didn't include gravy.
