Looking for a recipe for a slow cooker pot roast that falls apart, not one that you have to carve. Any suggestions?

It seems that every time I cook a pot roast in the slow cooker, it ends up being tender and delicious, but it has to be carved. I’m looking for a recipe for that classic “fall apart” or “shred apart” pot roast. The kind you can toss over mashed potatoes and toast and smother in gravy. The kind mom used to make.

Open to any suggestions, such as cut of meat, more liquid in slow cooker, etc.

And yes, I could just Google it, but if someone has a family heirloom recipe that they would live and die by, Google is useless.
